Title: Health Navigator
Reports to: Senior Health Navigator
Time: This is a full-time position with benefits (3-months probationary period at the beginning of contract).
Salary: Up to USD $55,000 / year
Adhikaar for Human Rights and Social Justice
Adhikaar, meaning rights in Nepali, is a New York-based nonprofit organization working with
Nepali-speaking communities to promote human rights and social justice for all. Established in 2005, Adhikaar is the only organization mobilizing the Nepali-speaking community in the New York area to raise our voices and take collective action against injustices on issues affecting us through organizing, community education, grassroots advocacy, leadership development, and individual assistance. We work in four program areas: workers’ rights, immigrant rights, language justice and access to healthcare. It is a fast-paced environment with many programs going on at once and the right candidate will thrive in such an environment.
Description
Advances access to healthcare New York State of Health/NYC Care H&H/ Keep NYC Covered to support the coordination of NYC Care services, by facilitating eligibility, enrollment and conducting public education activities and awareness about eligibility for available health insurance coverage options. To educate and enroll people through New York State of Health and NYC Care H&H. The Health Navigator will support other program areas as needed. The Health Navigator will report directly to the SeniorHealth Navigator for the overall work, outreach and work closely with other relevant staff, fellows and interns as needed.
Responsibilities:
Coordinate
NYS Health Navigator
- Understand the overall eligibility requirements for the health insurance program through the NY State of Health.
- Manage and educate all enrollment,renewal updates/changes on health insurance program through the NY State of Health,
- Provide referrals to Health Insurance provider or Marketplace, for any grievance, complaint, or questions regarding their health plan, coverage, or a determination under such plan,payment and PCP,
- Assure that application is accurately completed,adhering to submission deadline dates,
- Meet productivity and quality performance targets,
- Compile data/story from members with health care issues.Attend internal and external trainings and meetings,
- Support in regular reporting and tracking of programmatic deliverables and progress,
- Regularly track and update information of members inquires accurately in google sheet, and Powerbase,
- Provides information in a manner that is culturally and linguistically appropriate to the needs of the community.
NYC Care:
- Support the screening process/appointment for the enrollment to NYC Care,
- Follow up with members for NYC Care Program eligibility,
- Connecting people to the Financial Counselor, H&H, NYC Care,
- Attend Monthly Meeting with the MOIA, NYC Care,
- Support regular reporting and tracking of programmatic deliverables and progress,
- Regularly track and update information of members inquires accurately in google sheet, and Powerbase,
- keep all the information gathered in the course of your work confidential. No information may be discussed with anyone outside of appropriate Adhikaar personnel or their designees.
Outreach activities
- Lead community outreach, events and other activities for Health Care program
- Create outreach materials, fliers, event information and social media events.
- Facilitate meetings with information and health navigators services. Coordinate meeting Internal/external programs ensuring accurate information and health navigators services.
Other Responsibilities
- Support Adhikaar’s program staff with ongoing campaigns
- Provide language assistance
- Provide support for Adhikaar’s special events
- Support Adhikaar’s fundraising efforts
QUALIFICATIONS:
Required
- Demonstrates commitment to Adhikaar’s mission of human rights and social justice.
- Experience providing individual services and advocating for individuals.
- Ability to work collaboratively in a fast-paced work setting and follow the office mandated office Covid-19 protocols,
- Demonstrated ability to work with diverse populations and a wide variety of professional backgrounds and experiences,
- Proficient telephone Skills, Excellent verbal communication and listening skills,
- Ability to work in a team as well as lead independent projects,
- Competence in Microsoft Office, Google Drive, as well as social media tools,
- Must be based in New York City,
- Bi-lingual: Nepali and English required; additional skills in Hindi,Urdu/or Tibetan preferred.
